Accepted Insurance
I currently accept Aetna. I am working on getting credentialed with additional insurance companies, so please check back to see if your insurance is covered in the future.
If you would like to book a massage using your insurance, please select the Insurance Massage option when booking online. Prior to your first insurance appointment, you will need to:
fill out the intake form that will be emailed to you after booking
email a copy of your insurance card to kaylaannmassage.pllc@protonmail.com
email (kaylaannmassage.pllc@protonmail.com) or fax (+12534491892) a referral or prescription from your doctor
If I do not receive the 3 items listed above prior to your appointment (I need roughly 24 hours notice), I will need to reschedule your appointment to a later date once your benefits and referral have been confirmed.
